Overview
This standard identifies the requirements when collaborating with others to support individuals. This includes promoting the contribution of others to supporting individuals, working with them to achieve outcomes and empowering them to review the effectiveness of the support they provide
Performance criteria
You must be able to:
- identify and agree the role and responsibilities of yourself, others and individuals in collaborative working
- work in partnership with individuals and others to agree aspects of care or support collaborative working will involve
- lead on the provision of information to support others to meet the preferences and needs of individuals
- empower others to access resources, support and develop skills required to contribute to the outcomes of individuals
- work in partnership with individuals and others to identify and address any questions, concerns or risks in collaborative working
- promote the rights of individuals and others to ask questions, make decisions or change their mind
- empower others to participate in agreed care or support for individuals
- provide individuals and others with opportunities to feedback on how collaborative working meets the preferences and needs of individuals
- empower individuals and others to review and update outcomes of collaborative working and address any concerns
- take responsibility to ensure any changes required to collaborative working are implemented
- complete records and reports required for collaborative working with others to support individuals
- reflect on how you carried out your role and responsibilities when collaborating with others to support individuals
