Overview

This standard outlines the requirements when participating in partnership working to support individuals to access personalised care or support packages. This includes working in partnership with individuals and others to assess their preferences and needs, then working together to plan, implement and evaluate personalised care or support packages.


Performance criteria

You must be able to:

  1. identify and agree the roles and responsibilities of yourself and others in the assessment, planning and implementation processes for care or support packages for individuals
  2. work in partnership with individuals and others to access information on services that will address preferences and needs for care or support packages 
  3. empower and support individuals and others to review, identify and select preferences from options available 
  4. work with individuals and others to access and use agreed care or support packages
  5. carry out your roles and responsibilities in the assessment, planning and implementation process
  6. empower and support individuals and others in the assessment, planning and implementation process 
  7. encourage individuals and others to participate in reviews to feedback on how care or support packages have met their preferences and needs 
  8. provide your feedback on how care or support packages have met the preferences and needs of individuals
  9. work in partnership with individuals and others to identify and address any changes to care or support packages 
  10. complete records and reports required for promoting care or support packages through partnership working
  11. reflect on how you carried out your roles and responsibilities when promoting care or support packages through partnership working
