Overview
This standard identifies the requirements when you support individuals to monitor health conditions. This includes supporting individuals by carrying out your agreed role to monitor the health condition of individuals, identifying any changes and completing records and reports.
Performance criteria
You must be able to:
- identify and agree the roles and responsibilities of yourself and others to monitor the health conditions of individuals
- establish consent and access information on the health conditions and monitoring requirements of individuals
- work in partnership with individuals and others to identify how health conditions should be monitored and reported on, in line with individual’s preferences, needs, care or support plan, and specific monitoring requirements
- work in partnership to identify support individuals need to monitor health conditions
- support individuals to identify and access information, support or resources they need to monitor health conditions
- encourage and support the independence of individuals to monitor health conditions
- monitor health conditions in line with the preferences, needs, care or support plan, and specific monitoring requirements of individuals
- follow the required actions when there are changes in the conditions of individuals
- contribute to feedback to identify good practice and changes required to support individuals to monitor health conditions
- contribute to records and reports required to support individuals to monitor health conditions
- reflect on how you carried out your roles and responsibilities to support individuals to monitor health conditions, with feedback from others
